Medical Forms

Scouting America asks every participant in every activity to have a current medical form, called the Annual Health and Medical Record, along with a copy of their health insurance card.

Who needs a form

A new form is needed every year. That means a form for:

  • Every Scout
  • Every registered leader
  • The adult partner of every Lion and Tiger (kindergarten and 1st grade)
  • Anyone in your family who comes along to an outing or campout, like Cub O’Ree

Which form do you need?

Parts A and B: most Pack 214 activities

Use this for den meetings, pack meetings, day events, and trips and campouts shorter than 72 hours, including Cub O’Ree. No doctor’s signature is needed.

Parts A, B, and C: events 72 hours or longer

Like Webelos Adventure Camp. Part C is a physical exam signed by a doctor, so plan ahead; appointments can take time to get.

Not sure which one applies? Ask your den leader or email [email protected].

Filling it out

  • Fill out every section of each part, and sign where the form asks for a parent or guardian.
  • Include the month and year of every vaccination.
  • Attach a copy of the health insurance card, front and back.
  • Print it and turn in the paper copy.

When and where to turn it in

WhoWhen to turn it in

Bring forms to the Pack table, which is at every den meeting and pack meeting. If you already turned in a form this year, you’re all set.

How we keep your information

Forms are kept on hand at Pack events, never stored digitally, and will be returned to your family.
