Medical Forms
Every scout and adult needs a current medical form on file, along with a copy of insurance cards. A new form is required every year. Which version you need depends on the event, most Pack 214 activities use Parts A & B, but extended or high-adventure events like Webelos Adventure Camp require Part C as well. Most Pack 214 Events
Annual Health and Medical Record — Parts A & B. Use this for den meetings, pack meetings, day events, and most overnight events including Cub O’Ree. No physician signature is required for Cub Scout activities.
Webelos Adventure Camp & Extended Events
Annual Health and Medical Record — Parts A, B & C. Required for Webelos Adventure Camp and similar extended or high-adventure events. Part C requires a physician’s signature, so plan ahead, it can take time to schedule.
Bring a completed, printed copy with you, along with a photocopy of insurance cards (front and back). Forms are kept on hand at events, not stored digitally, and returned to families afterward.
